Chronic Diseases Clinics

These clinics are for patients suffering from diseases such as:

• asthma
• chronic obstructive pulmonary disease (COPD)
• diabetes
• coronary heart disease
• stroke
• hypertension (high blood pressure).

Patients will be invited to attend for an annual appointment with one of our experienced practice nurses as well as being seen for ongoing care with a doctor.
